The Office of Student Affairs and Admission is here for you to answer questions or clarify any details regarding the admission or enrollment process. Financial Aid
Once you have submitted your Financial Aid Application and all required documents the Financial Aid Office will examine them, then the award process will begin. The goal is to provide your financial aid award letter one month prior to the start of classes.
Award notifications and instructions for the on-line master promissory note will be mailed to the address you provided on your admissions file. Read the instructions included with the paperwork and complete and sign as instructed. The award note is to be returned to the Financial Aid Office (a copy is provided for your records). Please direct any questions you may have regarding your financial aid to Kathy Wright, at ext. 7490. New Student Orientation / Pre-matriculation Program
New student Orientation begins Thursday, July 29, 2010, at 8:30 am. All new students are REQUIRED to attend. Dress will be business casual.
Verified Credentials New Student Background Check
New students need to visit the Verified Credentials site and fill out the forms online. First year students, as well as incoming students and third year students that are starting their clerkships all need to complete the web form. Click on the link below. No other background checks will be accepted. Please have this completed by July 9, 2010. See https://client.VerifiedCredentials.com/OCPM/.

Drug Testing
All incoming students are required to have a drug test through Quest Diagnostics. The drug test must be conducted between the dates of June 14, 2010 and July 14, 2010. Any tests taken outside of these dates will not be valid, and no tests taken at a center other than Quest Diagnostics will be accepted. You will receive the form you need to take to the center in the mail at the permanent address you have on file at OCPM in mid-May. DO NOT FILL OUT THIS FORM AHEAD OF TIME. YOU MUST FILL OUT THE FORM AT QUEST DIAGNOSTICS IN FRONT OF THE QUEST STAFF OR IT WILL NOT BE VALID.

Housing Fair 2010
OCPM will be hosting its annual housing fair on Saturday, June 12, 2010, 10:00 am - 12:00 pm at the Ohio College of Podiatric Medicine. Representatives from area housing complexes will be on hand to answer your questions, schedule tours and sign leases. Also, on hand, will be area bank representatives, if you are interested in opening a local checking or savings account.
Housing Letter
If you are an incoming student, and you need to provide proof to your leasing agent that you will be an OCPM student, please email Kristy or call (216) 916-7489 to obtain a housing letter.
Insurance Needs
Insurance needs are met through Case Western Reserve University and National Union Fire Insurance Company. Every student is required to have University Health Services through Case Western University for a fee per semester. In addition to this, every student is required to have major medical insurance. The Student Medical Plan is offered to every student through National Union Fire Insurance Company, as well, for a fee per semester. Students have the option of waiving the major medical insurance if coverage is provided by a parent or spouse, or if they have their own major medical insurance. The waiver option will take place during Orientation, and is done online. The fees are factored into each student's yearly budget. A reimbursement check will be provided to those students who sign a waiver before the given deadline. The coverage runs from August 1st - January 1st; and again from January 1st - August 1st.
Mandatory Student Insurance
Case Western Reserve University Health Services Forms:
All incoming students must fill out the CWRU Health Services forms in preparation for the start of school. THIS IS A MANDATORY PROCEEDURE TO CONTINUE YOUR ENROLLMENT PROCESS. ALL INCOMING STUDENTS MUST RETURN THIS INFORMATION TO CASE WESTERN RESERVE UNIVERSITY IN ORDER TO START SCHOOL. Additional Medical Requirements
All incoming students must provide a written statement from a certified health care provider indicating satisfactory health and documentation of the following immunizations prior to matriculation:
- Measles, Mumps, Rubella (MMR) - student must have vaccination and positive titer for Rubella
- Polio - student must have vaccination
- Varicella - student must have documentation of illness or vaccination and a positive titer
- Hepatitis B - student must have vaccination and positive titer
- If a student has had the complete series of 3 shots done elsewhere prior to their matriculation at OCPM, they must provide proof of the vaccination along with a positive titer. Otherwise, the student must receive the Hepatitis B vaccination and have a positive titer drawn on campus during their first year.
- Tetanus (Tdap) - student must have current vaccination
- Tuberculosis (TB) - student must provide documentation of a negative TB skin test
- The TB test is provided on campus annually.
Immunization records should be submitted to the Chair of the Department of General Medicine and/or the Coordinator of the Secretarial Services Office. The records can be mailed, faxed, or e-mailed to:

Rite of Passage
A white coat ceremony will take place on Sunday, August 1st, 2010. OCPM must have your official coat size prior to Orientation in preparation for the ceremony. Each student needs to provide their full name and coat size (Women's sizes 4-22; Men's sizes 38-52). The white coat size is the same as the seasonal jacket you purchase for the spring/winter, etc. Do not email a letter size such as S, M, L - it must be a number size! Internet Access Agreement Form & New User Name Registration
Please follow both of these links below. You need to fill out the new user name registartion form (it will be submitted electronically). Print the last page of the Student Conduct Agreement for Electronic Access to OCPM Networks & Resources Form. THIS IS A MANDATORY PROCEEDURE. You need only print the last page, fill it out, sign it, and mail by June 3, 2010 to:

New Student Checklist
What you need to submit before orientation on July 29th:
- Do background check online at Verified Credentials by July 9th, 2010.
- Mail mandatory student insurance form to Case Western Reserve University.
- Take drug test at Quest Diagnostics site between June 14 and July 14, 2010. You will receive paperwork for the test in the mail in mid-May. You MUST bring the paperwork with you for the drug test.
- E-mail Barb Strong coat size for Rite of Passage Ceremony by June 3rd.
- Submit written statement in regards to health and immunization documentation to Dr. Robinson or Barbara Strong
- Mail Internet Agreement Form to Garrett Gaston at OCPM by June 3rd, 2010 .
- If you were accepted on a conditional basis, make sure all of the appropriate information (test scores, final transcripts, etc.) are in by Orientation on July 29th.
- Update e-mail and mailing addresses as they change to Kristy.
